Support people transitioning from hospital or managing longer-term health needs with practical exercise planning.
How we help
DVA exercise physiology should feel practical, respectful and connected to the health needs and activities that matter in day-to-day life.
Exercise planning may support conditions such as diabetes, cardiovascular disease, arthritis, osteoporosis and chronic muscular conditions.
Programs can support reconditioning after hospital treatment for a major injury or illness where clinically appropriate.
Build movement confidence, routine and skills to manage your health condition more independently.
Progress can be reviewed and communicated back to your GP or referrer where appropriate.
What happens first
Your first appointment is about understanding your referral, card details, health condition, current capacity and goals. From there, we create a clinical exercise plan and review progress as you move through your treatment cycle.
DVA cards, referrals and treatment cycles
DVA access depends on your assessed clinical need, card type, referral and the condition being treated. EHF cannot approve DVA eligibility or funding, but we can help you understand what information is usually useful before your first appointment.
DVA describes Gold Card treatment as covering clinically required treatment for all medical conditions.
DVA describes White Card treatment as covering accepted service-related conditions and some mental health treatment.
A valid referral usually comes from your usual GP. Initial referrals may also come from a specialist, hospital doctor or discharge planner.
DVA allied health referrals generally last up to 12 sessions or 1 year, whichever ends first, with GP review for further treatment if needed.
